If someone dies at home

• Telephone the practice. One of our doctors will visit to confirm that death has taken place
• Contact a funeral director
• Arrange to collect the Medical Certificate of Death (usually from the surgery).
• Take the Death Certificate to the Registrar Office (together with the deceased's medical card and Birth certificate (if available) for the area in which the death took place. Alternatively, you can register by declaration at any convenient Registrar Office. In the latter circumstance, a Death Certificate will be posted to you a few days later.
• The Registrar will normally issue a Green "Death Certificate" for you to give to your funeral director - who will look after funeral arrangements.  The Registrar will also issue a white notification certificate to the Department for Work and Pensions. If you are dealing with the deceased's affairs, Certified Copies of the Death Certificate may be needed for which there is a charge.

If someone dies in hospital

• Collect the Medical Certificate of Death from the hospital.
• Take the Death Certificate to the Registrar Office (together with the deceased's medical card and Birth certificate (if available) for the area in which the death took place. Your funeral director will usually lease with the surgery regarding the additional certification required for cremation.

Support after Suicide Bereavement

Amparo is a free support service for anyone affected by suicide across Lincolnshire.

The service provides practical and emotional support after bereavement. The experienced liaison workers can assist anyone impacted in accessing the support they need, whilst helping with a range of practical matters such as dealing with police and coroners; helping with media enquiries; and preparing for and attending inquests.
